Cancellation Policy

We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24 hours notice. Please extend us this courtesy so that other patients may be seen.

The following form can be downloaded to your computer and printed at home. By filling out this form ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out this form, please do the best you can and our staff will assist you with your questions on the day of your visit.

Post-Operative Instructions

The following forms can be downloaded to your computer and printed at home to provide patients with generla post-operative instructions for some of our most common procedures. Dr. Kharrazi will provide specific instructions to each individual patient during an office visit as well.
